Lisbon, June 14,2024 (Lusa) - The Portuguese Association of Hospital Administrators (APAH) argued on Friday that hospital management boards should be made up of professionals with training and experience in the sector, pointing out that only 18% of their members are career managers.
"The ULS [Local Health Units] are public companies with budgets in the hundreds of millions of Euros and are of fundamental importance to the proper functioning of our society. This obliges us to be extremely demanding when appointing their managers, which unfortunately has not always been the case in the past," says the APAH.
The position of the hospital administrators follows statements made by the minister of health, Ana Paula Martins, in the Health Commission on Wednesday, when she said that leadership in health is "weak" and argued that "there has to be scrutiny, there has to be performance evaluation for managers".
For the APAH, it is necessary to make "a clear distinction" between two types of leaders, both generally referred to as "hospital administrators", but which have significantly different scopes and functions.
"It's one thing for career hospital administrators to be hired to run the departments, services or units of the ULS, but it's quite another for the members of the boards of directors (CA) appointed by the government to be the top management of the Local Health Units," it emphasises.
According to the association, some career hospital administrators are appointed to be part of boards of directors, but the vast majority are not.
"We understand that the leadership that the minister was referring to are the ULS Boards of Directors appointed by the governments," she says.
These boards are made up of six or seven members, two or three of whom are doctors and one or two nurses. One of the other members is appointed by the local councils in the region and another by the Ministry of Finance.
The remaining members may or may not be career hospital administrators, who currently make up around 18% of the members of these boards of directors, CAs.
"In any case, we shouldn't embark on generalisations," argues the association led by Xavier Barreto in an allusion to the minister's statements.
For the APAH, the vast majority of boards of directors are made up of "capable and committed members" who deserve recognition.
At the hearing, Ana Paula Martins announced the creation of a commission to audit hospital management boards, to which the association reiterated that "any evaluation process should associate responsibility with the degree of autonomy and decision-making capacity".
"If, perhaps, the evaluation focuses on those who can effectively influence the results of the ULS (namely because they decide on hiring or investments), then it would be better to evaluate the performance of the Ministry of Finance as the main body responsible for managing these national health service units," it argues.
In the statement, the APAH also welcomes the minister's intention to review the hospital administration career, because "a revised and upgraded career will certainly be an important asset, both for the operational management of the ULS and the creation of a solid recruitment base for their CAs".
