Lusa - Business News - Cabo Verde: Army truck accident that killed eight due to human error

Praia,May.25,2023(Lusa)-The Chief of Staff of the Cabo Verdean Armed Forces said today that the driver of the vehicle that overturned in April while fighting a fire, causing eight deaths, lost control of the truck and "panicked".
"There are these two factors. The human factor, and the contributory factor, which has to do effectively with the failure to change, to reduce or increase the gear. And having failed to make this change in gear, [the driver] understood he had to crash the truck into the rock to try and reduce the gear in which he was travelling", António Duarte Monteiro stated.
The rear admiral was speaking at a press conference in Praia to present the conclusions of the enquiry that the Chief of Staff of the Armed Forces (CEMFA) himself set up to investigate the accident.
At issue is the fire that broke out on April 1, in the Serra Malagueta and Figueira das Naus Natural Park on Santiago island, 50 kilometres north of Praia, which was coordinated by the Cape Verdean Civil Protection with support from the military and firefighters.
The following day, a vehicle of the Armed Forces crashed on its way to the site, causing the death of eight soldiers and injuring 23 others, with a park technician also killed in the combat operations.
"The driver lost control of the vehicle and after several attempts to change gear, the lever [of the gearbox] broke. He certainly panicked, because he couldn't change the speed or the gear of the vehicle, and deliberately decided to crash the truck into the rock to try and stop it", António Duarte Monteiro added.
"It is clear that in the contributory factor we said the failure in the procedure of changing the gear of the vehicle and the consequent loss of control of it after several attempts and which resulted in the breakage of the upper part of the lever," pointed out the rear admiral, recalling that the forensics found "no evidence" of braking.
The truck, which was transporting the second military contingent to support the fire-fighting effort, had left Praia on the morning of 02 April, with 31 military personnel, three in the cabin and 28 in the body, bound for Figueira das Naus.
When directly listing the conclusions of the enquiry, which was supported by experts from outside the Armed Forces, the CEMFA stated that the truck, of the Dong Fang make and manufactured in 2021, was "in operational conditions of use", with "no breakdown or anomaly", and that the road was dry and in good condition.
"This finding was confirmed with the expert work carried out on the brake system, steering and other organs of the vehicle that did not accuse any anomaly" he said, noting that the driver "was duly qualified to drive the accident vehicle" since 2017 and had been on duty for the previous 48 hours.
"The multisectoral expertise is of the understanding that the origin of the accident began in the collection of information by the driver about the road and the late decision and effective execution in relation to the curve and the necessary slowdown," he explained.
He added: "This failure to adapt speed to the conditions of the road - therefore a very tight bend - and of the vehicle - length of 8.5 metres, and in relation to the weight it was carrying, although it was weighing less than what is allowed to be carried - led to loss of control of the vehicle, and the driver decided to crash the vehicle into the rock, going around the slope of the rock, reaching a height of 4.6 metres from the road, and then overturn".
He also said that "analysing all the facts and evidence gathered during the investigation process", including the technical reports and testimonies, "it was concluded that the direct cause of the accident is the loss of control of the vehicle by the driver after the lever broke, associated with an incorrect institutional assessment that resulted in the decision to crash the truck into the rock, combined with a lack of evidence of brake use".
"The contributory factor is the lack in the procedure of changing the gear of the vehicle and consequent loss of control of the vehicle, after several attempts and which resulted in the breakage of the upper part of the lever," he pointed out.
According to the CEMFA, the survey also concluded on seven actions to be adopted by the Armed Forces to "increase operational safety in means of land transport", whose implementation "will begin immediately".
"Increase the continuous training of drivers, including theoretical and practical exercises based on various scenarios, in order to prepare drivers to face and react with discernment and cold blood to any type of breakdown with vehicles in motion. Implement training in defensive and evasive driving", he pointed out.
It will also include "transversal training of human resources that work in the area of maintenance, operation, driving and administration of vehicles", namely "through technical and scientific exchanges with institutions with expertise", as well as "providing general transport vehicles with better safety conditions for the transport of staff in the bodies".
Other measures include "equipping the vehicles of the Armed Forces, when operationally appropriate, with safety and control equipment, such as tachographs or GPS systems, as well as speed limiters.
"Implement procedures on the use of individual protection equipment for the protection of military personnel to be transported in open-top vehicles" and "equip the Armed Forces Central Workshop with better equipment to autonomously guarantee the repair and maintenance of its land transport means".
